    To ask the Secretary of State for Health, what assessment he has made of differences between bereavement support services for families who have suffered a stillbirth or neonatal death commissioned by clinical commissioning groups and such services provided by hospital trusts; and if he will make a statement.

    A report in 2015 on Term, singleton, normally-formed, antepartum stillbirth from Mothers and Babies: Reducing Risk through Audits and Confidential Enquiries across the UK found that 60% of parents currently receive a good standard of bereavement care but this is not the case for everyone and we are continuing to consider the actions that should be taken to improve bereavement care across England.

    NHS England has established a Maternity Transformation Programme Board, this will bring key partners together to oversee the implementation of a broad range of policies to deliver significant improvements to maternity care in England, including implementation of the recommendations of Better Births, Improving outcomes of maternity services in England (2016). The Transformation Programme includes work on supporting local transformation of maternity services, promoting best practice for safer care, increasing choice and reforming the payment system.

    In Delivering the Forward View: NHS planning guidance 2016/17-2020/21 localities have been asked to produce “Sustainability and Transformation Plans” to show how local services should transform and ensure they are sustainable over the next five years. As part of this, local health economies have been asked to plan how they will transform their maternity services in line with the vision outlined in Better Births, Improving outcomes of maternity services in England. NHS England will be reviewing how well commissioners are planning for delivery of this vision in signing-off plans; and how well those plans are being put into action and on an ongoing basis through its Clinical Commissioning Group (CCG) Assessment Framework, and annual Ofsted-style rating of each CCG on its commissioning of maternity services.

    To ask the Secretary of State for Transport, what training taxi and private hire drivers are required to complete to support blind and partially sighted passengers.

    Whilst no national mandatory requirements exist to provide training, we strongly encourage licensing authorities to consider requiring their taxi and private hire drivers to undergo training, ensuring that every passenger can be provided with a first class service.

    Well-designed disability awareness training, provided by the local authority, can help drivers to understand their legal duties and to equip them with the knowledge and skills to assist a range of passengers, including those who are visually impaired.

    We have been clear that we want to see all schools, over time, become academies. But our focus is on schools where standards need to be raised. So we will continue to convert all schools that are judged inadequate by Ofsted, and work with those that are coasting or otherwise underperforming to identify the support they need to improve. We expect that in some cases this support will be through conversion to become a sponsored academy. No new legislation is required for these purposes.

    The Department for Education expects to report on the review into the use of special guardianship orders by the end of the year.

    The findings of the review have been discussed extensively with an expert advisory group whose membership includes representatives from voluntary agencies for adoption, fostering and kinship care, local authorities, academics, ADCS and CAFCASS.

    To ask the Secretary of State for Health, what proportion of levels of blindness and low vision are classifiable as preventable.

    There is a range of information already available about levels of preventable sight loss. The indicator within the Public Health Outcomes Framework brings this together in a single place. The indicator is made up of four sub-indicators which will measure the crude rates of Age-Related Macular Degeneration, glaucoma and diabetic retinopathy, and of the rate of sight loss certifications per 100,000 population. Data has been collected since 1 April 2013.

    Baseline data for 2010/11 was published in November 2012, broken down by local authority and data providing a time series up to 2013/14 has now been published. It is available to search at:

    To ask Mr Chancellor of the Exchequer, how many evasion referrals were made to the HM Revenue and Customs (HMRC) Evasion Referral Team in each year since 2012-13; and how many such referrals were adopted as working cases by HMRC’s (a) Criminal Investigation, (b) Specialist Investigation and (c) Local Compliance Fraud business unit in each such year.

    The referrals are made by HMRC officers when they suspect or discover evasion. The process is designed to escalate this type of case to a specialist team for review. If the case is not adopted by one of these teams it is returned to the referring officer to continue the investigation.

    To ask the Secretary of State for the Home Department, how many people have been extradited from Pakistan to the UK in each of the last five years.

    Between 1 January 2010 and 31 December 2015 there were no extraditions from Pakistan to the UK.

    There is no bilateral extradition treaty between the UK and Pakistan and, unless a multilateral convention applies, extradition is on the basis of reciprocity.

    The Government will neither confirm nor deny whether it is considering or is involved in negotiations with a particular country regarding an extradition treaty.

    To ask Her Majesty’s Government, further to the Written Answer by Lord Ahmad of Wimbledon on 17 March (HL6801), what plans they have to introduce locally managed and self-financing schemes to deal with air pollution and the provision of urban bus services.

    We do not have specific schemes focused on self-financing urban bus services and air quality. However the Government is investing significant funding to help local authorities improve air quality and reduce emissions of harmful pollutants. This includes provision to bus fleets.

    The Department for Environment, Food and Rural Affairs (DEFRA) published an air quality plan for nitrogen dioxide in December 2015 setting out a comprehensive approach to meeting our ambition for the UK to have some of the cleanest air in the world, including by implementing a new programme of Clean Air Zones. Under the Plan, by 2020 the most polluting vehicles, including older polluting buses, will be discouraged from entering the centres of Birmingham, Leeds, Southampton, Nottingham and Derby. DEFRA has allocated funding to help these five cities implement Clean Air Zones. Additionally cities including Oxford, Norwich and Brighton have already taken steps to introduce zones to ensure the operation of cleaner buses in their areas.

    To ask the Secretary of State for Environment, Food and Rural Affairs, what funding (a) her Department and (b) the Environment Agency provided for the Climate Ready Support Service in 2015-16; and what assessment her Department has made of the performance of that Service in (a) 2014-15 and (b) 2015-16.

    Funding provided for the Climate Ready Support Service in 2015-16 was around £1 million. The Climate Ready Support Service Programme Board had regular overview of the performance of the Service. The majority of National Adaptation Programme actions to which the Service was contributing have now been completed.
